want to use http://tubesitessubmitter.com/ for video uploading, I want to send my tube site to the Tube Sites Submitter and they will send me lots of videos to my tube sites. How can I set up upload form script for this script.

Let's clarify this a bit

So some script will automatically submit some unknown videos into your db, am I correct ?

yes .. standard upload form like in tgp submit form

At present time we don't have upload forms as it involves a lot of settings\security checks on what users can upload and so on

But if you'd like to add videos from somewhere you can ask for a RSS or dump that can be updated as often as you want and the script will import videos from that RSS\dump using Import sets

that can't make via RSS ....

If you can get a dump like "video_url|description" - yes, you can upload it this way

nope I want videos from uploaders that use https://www.tubesitessubmitter.com/ not videos from sponsors

I can make a simple form for you, so you can use it at your sites
